Event Initiation Form

The Donor Relations team plans and executes event strategies that strengthen relationships among the university’s donors. Please use this form to request event strategy and logistical support from the Donor Relations team. This form is your first communication to initiate the planning process. Once your event is approved and budget dollars are allocated, you will be assigned an event manager who will set up an initial meeting with you.

Please note that you do not need to have every section filled out in order to request assistance – only fields marked with an asterisk (*) are required. We are happy to help you coordinate the details and design an event strategy that meets your goals and objectives.
